The brand-new Citrus Heights Health Center, opened in late 2023, offers both long- and short-term skilled nursing for those in need of 24-hour support in a medical setting. Residents are cared for by a team of skilled practitioners, while those recovering from illness or sickness have access to a wide range of therapy and services

Short-Term Rehab

The Citrus Heights Health Center offers best-in-class rehabilitative and respite care in a comfortable setting with hotel-like amenities and expert support to help residents recover from a stroke, heart attack, illness, or injury. Short-term rehab services are open to the general public, typically through Medicare/Medicaid and secondary insurance.

Rehabilitative Services

Residents in the short-term rehab program are supported by a dedicated case manager, who along with an interdisciplinary team of nurses, therapists, social services managers, and nutritionists works with your physician to create an individualized care plan. Among the services offered are:

  • One-on-one physical, occupational, and speech therapy
  • Neurological, cardiopulmonary, and stroke rehabilitation
  • Medical and post-surgical rehabilitation
  • Podiatry and wound care
  • Around-the-clock medical and rehabilitative nursing care

Eligibility and Fees

Short-term care at the Citrus Heights Health Center is open to individuals with or without Masonic affiliation, subject to the following requirements:

  • Referral from a health care provider
  • Three-day qualifying stay at an acute care hospital
  • Medicare medical insurance
  • Discharge goal of returning home
