Transitions: Quality Recovery in the Tri-City Community

That’s why the Masonic Homes opened Transitions short stay rehabilitative care services on their Union City campus. Today, members of the public in the Tri-City region and beyond can access the same dedicated care that Masonic Homes residents have enjoyed for more than 150 years.

Established in 2016, Transitions welcomes patients of all ages for short-stay rehabilitative care following surgery, illness, or injury. The program’s highly qualified staff includes a dedicated RN case manager and an extensive interdisciplinary team of doctors, nurses, therapists, social services managers, and nutritionists. Specialized services are offered to patients recovering from orthopedic, neurological, and cardiopulmonary surgeries as well as those who have experienced strokes. Around-the-clock medical and rehabilitative nursing care is partnered with expert pain management and assistance with personal care needs. One-on-one physical, occupational, and speech therapy sessions are available, and all patients enjoy a patient-to-staff ratio that is significantly better than the national average.
